Coder will assigned to perform coding services on either Inpatient, Outpatient and/or Ambulatory Procedure Visit (APV) medical recor! ds documentation, and provide educational feedback to coders and physician’s addressing inappropriate patterns of documentation of records audited.

This is a Service Contract Act (SCA) covered position.

Inpatient Coding Duties and Responsibilities

Applies ICD-9 codes for diagnoses and procedures (Diagnostic Related Group is automatically assigned by the grouper software) for inpatient stays.

Identifies and extracts information from medical records (paper or electronic) for special studies and audits, internal and external

Interacts with MTF staff to ensure documentation is clear and supports coding assignments. Educates MTF staff through individual or group in-services and training sessions.

Maintains a delinquency report of missing documentation for inpatient records in order to facilitate completion of work within the required Air Force thresholds.

Ensures all required component parts of the inpatient medica! l record that pertain to coding are present, accurate and comp! ile with DoD, Air Force and JCAHO requirements.

Responsible for assignment of accurate E&M, ICD, CPT, and HCPCS codes and modifiers as documented by the attending physician’s IBWA/rounds encounter during the patient hospital stay.

Identifies and extracts information from medical records (paper or electronic) for special studies and audits, internal and external.

Maintains a delinquency report and monitor lost IBWA/rounds records for reporting to MTF management.

Outpatient Coding Duties and Responsibilities

Responsible for assignment of accurate E&M, ICD-9, CPT and HCPCS codes and modifiers from medical record documentation lost IBWA/rounds records for reporting to MTF management

Identifies and abstracts information from medical records (paper or electronic) for special studies and audits, internal and external

Interacts with MTF staff to ensure documentation is clear and supports coding assignments. Educates M! TF staff through individual or group in-services and training sessions

Maintains a delinquency report of missing records in order to facilitate completion of work within the required Air Force thresholds

Ambulatory Coding Duties and Responsibilities

Responsible for assignment of accurate E&M, ICD-9, CPT and HCPCS codes and modifiers from medical record documentation into the Government computer systems

Identifies and abstracts information from medical records (paper or electronic) for special studies and audits, internal and external

Interacts with MTF staff to ensure documentation is clear and supports coding assignments. Educates MTF staff through individual or group in-services and training sessions

Maintains a delinquency report of missing records in order to facilitate completion of work within the required Air Force thresholds

Works closely with the Coding Supervisor/auditor during audit process

E! nsures all required component parts of the medical record that pertain ! to coding are present, accurate and compile with DoD, Air Force and JCAHO requirements

Works with Coding Compliance-Editor software to ensure records are accurately coded

Qualifications

Registered Health Information Technologist (RHIT) or Registered Health Information Administrator (RHIA)

Certified Professional Coder (CPC), Certified Professional Coder-Hospital (CPC-H), Certified Coder Specialist (CCS), Certified Coder Specialist â€" Physician (CCS-P) are required with a minimum of three year experience in the outpatient setting (physician’s office or ambulatory care centers) within the last five years.

Education and/or certification from an accredited institution recognized by the American Health Information Management Association (AHIMA) and/or American Academy of Professional Coders (AAPC)

Minimum of three years experience i! n the inpatient, outpatient or ambulatory setting within the last five years is required or a minimum of two years if experience was in a Military Treatment Facility

Must be able to attain and maintain a National Agency Check with Inquiries (NACI) required

Knowledge of anatomy/physiology and disease process, medical terminology, coding guidelines (outpatient and ambulatory surgery), documentation requirements, familiarity with medications and reimbursement guidelines; and encoder experience.

Excellent computer/communication skills for provider and staff interactions

Ability to handle multiple projects and appropriately prioritize tasks to meet deadlines.

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.

Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Additional Requiremen! ts

Knowledge of current and developing issues and trends in me! dical coding.

Attention to detail and data entry accuracy is essential

Familiarity with unique military healthcare systems and procedures

Use of one or more computer programs and monitors may be required daily.

Must be able to sit, verbally converse and work on a computer for long periods of time with or without reasonable accommodation

Requirements may include prolonged walking, standing, sitting or bending. Carrying or lifting of medical records may be required daily.

Frequent use of hands/fingers across keyboard or mouse or handling other objects.

Work Environment

Coders will be located in the centralized Health Information Management coding offices. Computer, books, references material provided to coder to fulfill task.
