VAMC Salem, in addition to the main medical center in Salem, Virginia, operates Community Based Outpatient Clinics (CBOC's) in Lynchburg VA, Staunton VA, and Wytheville VA, and is also responsible for Employee Health services in the Medical Center. VA uses a state of the art electronic medical record that contains all clinical information related to VAMC outpatient and inpatient care and remote data from other VA and DOD sites. Each Primary care team practices and coordinates patients' care with support of Pharm D., social worker, dietician, integrated mental health professionals, and a broad range of medical, surgical, and mental health specialists either on site or by telemetry.
The Primary Care GAP position is designed to fill in the gaps in continuity of care that may arise due to anticipated or unanticipated absence of the designated Primary Care Provider. Thus the GAP provider performs all the duties of the Primary Care provider until the designated provider is available to resume management of his/her panel of patients.

DUTIES :
Provides patient centered primary care medical service to male and female veterans in collaboration with a dedicated RN and LPN.
Manages chronic conditions.
Evaluates and treats intercurrent medical conditions.
Screens for onset of new health related conditions
Collaborates with other primary care teams and providers when needed.
Practices and coordinates patients' care with support of Pharm. D, social worker, dietician, integrated mental health professionals, and a broad range of medical, surgical, and mental health specialists either on site or by telemetry.
Uses a state of the art electronic medical record that contains all clinical information related to VAMC outpatient and inpatient care and remote data from other VA and DOD sites.
Performs occasional minor office procedures common to general internal medicine and family practice skills.
May, at times, need to travel to CBOC's to provide patient care service as well as provide coverage in the Employee Health clinic.
QUALIFICATIONS REQUIRED:

U.S. Citizenship Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. Current, full and unrestricted license to practice medicine or surgery in a State, territory, or Commonwealth of the United States, or in the District of Columbia
