Care Management Monitor (CMM) for Navy Reserve Medical Entitlements Determinations (RMED) will ensure Navy Reservists are engaged with th! e appropriate health care and case management services and progress in an effective and timely manner to resolution of issue(s). This individual is responsible for ensuring that the approximately 245 Wounded, Ill and Injured Navy Reservists in the process of demobilizing have been provided with referrals for follow-up care either at the Veterans Administration or with a TRICARE provider in their area. Responsibilities include:
- Maintain surveillance of PH, TBI and related health care needs of Navy Reservists on Medical Hold or Line of Duty benefit.
- Verify that assigned Sailorsâ medical issues are effectively addressed through the Limited Duty Coordinators, unit medical staff or assigned medical case managers and engage situations to rectify deficiencies and arrange appropriate local care management support. This may entail engaging Military Medical Support Office (MMSO) and coordinating with BUMED, TRICARE, or VA to ensure appropriate case manage! ment is provided and in some instances initiating a referral f! or a Recovery Care Coordinator. In some situations, the CMM may need to coordinate with memberâs unit to send a member to a MTF or a Navy Reserve unit when needs are inadequately met, medical board processing is needed, or a second opinion evaluation by a MHS provider is requested.
- Have Reservistâs local care support personnel (LDCs, unit corpsmen. medical case manager) verify that that all membersâ ailments have been brought forward and are being appropriately addressed.
- Ensure that assigned Sailors adhere to the appropriate medical administrative requirements (e.g. Limited Duty status, Abbreviated Medical Evaluation Board Reports, Narrative Summary, disability processing). This may include engaging memberâs command or clinical support staff when proper administrative procedures for Medical Hold and Line of Duty Benefit are not met.
- Ensure that assigned and supported Sailors adhere to appropriate timelines for resolution of condition(s) o! r processing for disability. Identify Sailors demonstrating poor compliance with follow up treatments or evaluations to Navy Reserve Force Surgeon. For cases referred to the Integrated Disability Evaluation System, track compliance with time requirements for various phases and advise Navy Reserve Force Surgeon when timelines are not met.
- Assist Navy Reserve Force Surgeon in providing education outreach to reserve unit leadership, Limited Duty Coordinators, unit medical staff and other designated parties regarding Medical Hold, Line of Duty Benefit, Medical Boards and IDES.
- Access and utilize AHLTA or other data bases as assigned.
- The CMM shall engage Patient Administration Staff, Medical Boards Offices, Military Services Coordinators, and Unit leadership as well as Medical Case Managers to facilitate Medical Board and disability evaluations for assigned Sailors.
- In acutely deteriorating situation when urgent action is required, the CMM sh! all intervene to directly coordinate care until the appropriate Medical! Case Manager(s) can engage.
- When requested by Navy Reserve Force Surgeon the CMM shall conduct clinical care reviews on designated cases and provide a written synopsis of care issues/needs and propose courses of action to resolve issues. When situations necessitate, a verbal report shall be provided in lieu of written only if authorized by the Navy Reserve Force Surgeon.
- As workload permits, the CMM when requested by the Navy Reserve Force Surgeon to review assigned cases for Traumatic Servicemenâs Group Life Insurance (TSGLI) eligibility and other medically related benefits programs.
Bachelor in Science Degree in Nursing from an accredited program.
Required License.
A current active state license and certification by the American Nurses Credentialing Center.
Required Certifications.
Certification by the American Nurses Credentialing Center and one of the following certification! s:
1. Commission for Case Management Certification (CCMC): the Certified Case
2. National Academy of Certified Care Managers (NACCM): Care Manager Certified (CMC)
3. Other Certification if approved and authorized by the Regiment Surgeon
Within four months of employment meet the Case manager Training and certification requirements outlined in the Navy Medicine Clinical Case Management Instruction (BUMEDINST 6300.17).
Required Experience.
Medical care management experience in the Military Health System.
Experience with VA Health System for medical care coordination.
Experience as a military reservist or experience performing case management for reservists is desirable.
Required Training.
Must demonstrate training that is current with licensing requirements.
