Responsibilities:
Contract personnel shall input data abstracted from the medical record into CHCS utilizing appropriate data codes and system commands for data entry. Performs edit checks on data entered prior to transmittal and corrects errors as indicated.

Enter the appropriate diagnostic ICD-9; CPT; and /or HCPCS; and modify into the Ambulatory Data Module (ADM) of the Composite Health Care System (CHCS)

Code principal diagnosis, co-morbidities, complications, therapeutic and diagnostic procedures, supplies, materials, injections, drugs, etc. with ICD-9-CM,

Code transcribed electronic consults using specific E&M consult CPT codes.

Identify additional diagnoses/procedures to optimize reimbursement; i.e., complication, co-morbidities, therapeutic procedures and diagnostic proce! dures.

Use electronic medical records systems primari! ly as the mechanism to review and audit encounters; print, copy, pick up/deliver encounters from clinics that are not using the electronic document system.

Review, Analyze and verify component parts of the medical records and ambulatory encounters for consistency, accuracy of diagnosis and completeness for coding purposes using established criteria and regulations that must conform to Health Care Financing Administration and Department of Defense Coding Guidelines.

Examine all documents in the record for authorized signature and patient identification to ensure all documents contain sufficient documentation to support the diagnosis and treatment administered, and that results obtained are adequately described.

Review and identify the correct principal diagnosis and principal procedure on encounters based on physician’s attestation record documentation and established sequencing rules and guidelines.

Contact medical staff for clarificati! on and additional information to ensure accurate coding. Interprets and verifies the reason for the encounter, including cause(s), principal diagnosis, principal procedure (s) performed and significant related diseases to assure record contents meet JC and Army regulation requirements for the highest attainable quality.

Interact closely with Health care providers and technician to educate and improve clinical documentation as it applies documentation of patient encounters. Provide feedback to providers using the review and audit to improve physician documentation practices.

When documentation of the medical record is not adequate to identify the appropriate code, the contract employee shall indicate the specific information concerning the additional documentation required. The contract personnel shall provide documentation training with clinic providers/staff as directed by the supervisor or appointed representative.

Responsible for validating cod! es of the diagnosis evaluation and management, procedures or any other ! codes required for the complete and accurate preparation for the SADR and to support the Uniformed Business Office (as directed) to ensure they have the documentation to submit a claim.

Correct inappropriate coding performed by providers using the CHCS Legacy System, Coding Compliance Editor (CCE), and modifying the ADM record.

Clinic encounter unable to be coded due to administrative errors shall be returned to the specific Departments for corrections. Codes for APV encounters may be indicated in the APV record as a cover sheet. The specific department administrative staff shall make corrections and return the case to the contact personnel for final coding.

Modify inpatient rounds, cancel appointments, other data entry functions, required for the Industry Based Workload Alignment (IWBA) which do not meet the clinical documentation and educate the physician as to the reason the round visit does not count.

Contract personnel shall inform t! he government supervisor of any corrections that need to be made to the medical records or computer systems if errors are detected. The supervisor shall then forward discrepancies to the provider and correct as appropriate.

Assign the correct diagnostic or procedure code to each ambulatory episode ambulatory patient treatment utilizing accepted ICD-9-CM, CPT, and HealthCare Common Procedures and Coding system.

Complete a minimum of 120 ambulatory encounters per eight hour work-day. If coding ambulatory procedure visits (APV), the expectation is 25 records per eight hours work-day. If coding inpatient rounds, the expectation is 50 encounters per eight hours work-day. If coding inpatient encounters, the expectation is 20 encounters per eight hours work-day.

Follow the billing and documentation requirements out lined in the Center for Medicare and Medicaid Studies (CMS) Guideline for Billing Resident Services in a Teaching Hospital. These guidelines ! must be followed as pertains to coding, documentation and compliance gu! idelines..

Enter Privileged provider information as ASN (Additional Provider) field to comply with HCPFA billing requirement.

Use locally designed Clinics Use Only (CUO) field of ADM to capture coding otherwise limited by DoD system design i.e., modifiers.

Coding of all encounters shall be completed immediately upon receipt of encounters. Encounters will be completed within 72 hours of date of care for outpatient encounters, 15 days for APV encounter, 30 days for inpatient encounter and 30 days for inpatient rounds.

Prepare reports: productivity, end of day (EOD) ADM, physician findings, CHCSII results, quality reports and attend meetings as specified.

The contract personnel shall be responsible for safeguarding both the record and its informational content against loss, defacement and tampering, and from use by unauthorized individuals.

Qualifications:
a. Contract personnel must provide copies of crede! ntials and a resume to include three years of coding experience in an ambulatory and inpatient setting within the last five years for all personnel prior to assignment at the Fort Belvoir Community Hospital.

b. Annual reporting of accreditation certification is a requirement and shall be provided without request. Reference from previous healthcare facilities will be made to determine compatibility with the Fort Belvoir Community Hospital needs.

c. Certification through the American health Information Management Association (AHIMA) or the American Academy of Professional Coders (AAPC).

d. Coders are required to have extensive knowledge of the official ICD-9-CM, CPT and HCPCS Coding Guidelines for coding and reporting.

e. Contract personnel shall read, understand, speak and write English.

f. Coders must possess medical and technical terminology with knowledge of specialized vocabulary peculiar to a variety of subjects in the field ! of medicine and surgery to include the following services:
